Do you have split ends? Because hair that is split at the end is weaker and tends to break off more easily, say, when you’re brushing it. To check, examine the tips of your hair and if it’s split into two, there you have your answer. The only way to cure split ends is to cut hair above the level of the split.
Keep in mind that hair dryers, straighteners etc can make your hair more prone to breakage and dryness. Use a good shampoo and conditioner. And towel-dry your hair after washing it – never comb too aggressively or it will all fall out.
If the problem gets too serious, see a doctor.

Or you could be using shampoos that contain sulfate, which causes hair loss. If you are using shampoos with these, you should maybe switch to using Dove shampoos which is sulfate-free.
